Tylenol vs Advil: which one for which pain? A pharmacist's plain-English guide

- Tylenol (acetaminophen) is the gentler, more universal choice. Best for fevers, headaches, gentle pain, and people with sensitive stomachs.
- Advil (ibuprofen) is an anti-inflammatory. Best for muscle and joint pain, menstrual cramps, dental pain, and any pain where swelling is part of the picture.
If you only had room in your medicine cabinet for one — most pharmacists would say Tylenol, because it's safer for more people. But each is genuinely better than the other for different situations. Here's how to choose without overthinking it.
What each one actually does
Tylenol (acetaminophen) works centrally, in the brain, to dull the perception of pain and lower fever. It does not reduce inflammation — that's the key difference. Because it doesn't act on the stomach lining or kidneys directly, it's gentle on most people, including those with ulcers, kidney issues, or who are on blood thinners. It can be taken on an empty stomach.
Advil (ibuprofen) is a non-steroidal anti-inflammatory drug (NSAID). It blocks the chemicals (prostaglandins) that cause both pain and the inflammation behind it. That's why it's particularly effective for any pain involving swelling — strained muscles, sprained ankles, menstrual cramps, dental pain, and arthritis flares. The trade-off: NSAIDs can irritate the stomach lining and stress the kidneys, which is why we always recommend taking ibuprofen with food.

Here's how to think about it:

The "Tylenol AND Advil" trick
For severe pain — post-surgery, dental work, a serious migraine — alternating Tylenol and Advil every 3 hours often works better than maxing out one alone. Here's the schedule:
- 8 a.m. — Tylenol
- 11 a.m. — Advil
- 2 p.m. — Tylenol
- 5 p.m. — Advil
- ...and so on
Both medications have different mechanisms, so they don't compete for the same liver/kidney pathway, and you stay below the daily maximum of each. Many dentists, ER doctors, and pain specialists in Kitchener-Waterloo recommend exactly this approach for moderate-to-severe pain at home.

How much is too much
This is where things get serious. Both medications have ceilings.
Tylenol (acetaminophen) maximum for healthy adults: 4,000 mg per 24 hours (8 extra-strength tablets), with at least 4–6 hours between doses. The danger is liver damage from accidentally exceeding this — and acetaminophen is hidden in lots of products: NeoCitran, cough/cold remedies, prescription pain pills (Tylenol #3, Percocet). Always read the label and total your daily acetaminophen across all products. That accidental overlap is the most common cause of acetaminophen-related ER visits.
Advil (ibuprofen) maximum over the counter: 1,200 mg per 24 hours (6 Liqui-Gels), with food. Above that, you need a doctor's supervision. People with kidney disease, heart failure, uncontrolled high blood pressure, stomach ulcers, or pregnant women in the third trimester should avoid ibuprofen unless their doctor approves.

Q: Is Aleve different from Advil?
Aleve (naproxen) is also an NSAID, similar to Advil but longer-acting (12 hours vs Advil's 4–6). Same precautions, same situations.
Q: My toddler has a fever. Which one?
Under 6 months — use Tylenol Infant only; ibuprofen isn't recommended for babies under 6 months. After 6 months, both are safe at the right weight-based dose.
Q: I had a few drinks tonight. Is Tylenol safe?
Be cautious. Acetaminophen + alcohol is harder on the liver than either alone. If you've had several drinks, Advil with food is generally safer for that one night — but check with us if you have liver concerns.
Q: Can I take Tylenol for back pain?
Yes, but Advil usually works better for back pain because muscle spasm and inflammation are part of it. Try Advil first if your stomach can handle it.
