What Are Cold Sores?
Cold sores (herpes labialis) are caused by herpes simplex virus type 1 (HSV-1). Once infected, the virus stays dormant in nerve tissue and can reactivate with triggers such as stress, sun exposure, illness, or fatigue. Common symptoms include:
- Prodrome phase: tingling, itching, or burning on or around the lips — this is the ideal time to start treatment
- Blister phase: small fluid-filled blisters that cluster on or around the lips or mouth
- Healing phase: blisters crust over and resolve, typically within 7–10 days without treatment
- Outbreaks can recur throughout a person’s life

Antiviral Medications for Cold Sores
Ontario pharmacists can independently prescribe antiviral agents for herpes labialis, including:
- Acyclovir (Zovirax®) — oral or topical antiviral
- Valacyclovir (Valtrex®) — oral prodrug of acyclovir with improved absorption, often the preferred choice for episodic treatment
Timing is everything. Antivirals work best when started during the prodrome stage — ideally within 48–72 hours of the first symptom (tingling or burning). Don’t wait for blisters to form. The sooner treatment begins, the shorter and milder the outbreak.

Is this covered by OHIP?
Yes. The pharmacist assessment for minor ailments, including cold sores, is billed directly to OHIP. There is no out-of-pocket cost for the visit itself. Prescription drug costs are covered according to your regular drug benefit plan (ODB, private insurance, or out-of-pocket).
What antiviral will I get?
Your pharmacist will choose the most appropriate agent based on your medical history, current medications, and severity of symptoms. Valacyclovir is commonly prescribed for episodic outbreaks. Acyclovir may also be recommended. Both are effective antivirals for herpes labialis.
How soon should I start treatment?
